When a young medical student receives a life-saving heart transplant, she uses her second chance at life to learn more about the young man who was her heart donor. But after she travels to a remote village to pay her respects, she discovers a string of unsettling disappearances that seem to be tied to her donor’s grieving family, and unintentionally springs a deadly trap.

In 2023, Clara (Sopelana), a student doing her residency in Madrid, suffers what would have been a fatal heart attack had she not been the lucky recipient of a serendipitous organ donation. As she recovers, she’s overcome by the urge to find out who her donor is — but organ donors’ identities are strictly anonymous in Spain. With some clever internet sleuthing, Clara manages to find him anyway: His name is Carlos Ferrer (Carlos Cuevas), a young man with brittle bone disease who died in a car accident. Clara feels compelled to call Carlos’s mom, Marta (Ituño), and offer her condolences. Marta, in a surprising turn of events, invites Clara to visit over the weekend and spread Carlos’s ashes with the family.
The day Clara arrives, she meets Marta’s other son, Juan (Nieto), a local police officer who treats her with polite suspicion, along with Rafael (Massagué), Juan’s boss, who tells her a disturbing tale about the Ferrers: Marta’s husband, Miguel (García), also a police officer and Rafael’s best friend, disappeared in 2005 when the boys were children — and his body was never found. The case bore a striking similarity to a handful of unsolved cases stretching back 20 years. Clara is unnerved by the morbid story with all its coincidences, but thinks little of it until panic spreads through the village after Carlos’s funeral. A young infant has been kidnapped, just like Miguel and others before him, and something in Clara — perhaps the part of her that’s Carlos — compels her to stay and help Juan solve the case.
